We are seeking a Primary Care Nurse Practitioner to provide comprehensive medical care to residents in a nursing home facility located in Pittsburgh, PA . The ideal candidate will demonstrate strong clinical judgment, compassion, and the ability to work collaboratively within an interdisciplinary team to ensure the highest standards of care.

Key Responsibilities:

• Manage all primary care consultations for residents
  
• Conduct detailed histories and physical examinations
  
• Perform medication reviews and prescribe medications as appropriate
  
• Evaluate acute medical issues and establish follow-up care plans
  
• Formulate informed diagnoses based on medical knowledge and patient history
  

Qualifications:

• Valid Nurse Practitioner license in the state of Pennsylvania
  
• Board Certification required
  
• Previous experience in long-term care preferred
  

Schedule:

• Full-Time, Monday through Friday
