Credential Requirements:
● Valid LPN License
● Up to Date CPR Certification
● TB Test completed with the past 12 months
● Physical completed within the past 12 months
● A valid Driver's license or state Identification
● Social Security Card or Birth Certificate
● COVID Vaccine is required
