Key ResponsibilitiesImplement and manage personalized patient care plans.
Provide mobile support to the Lebanon, PA community.
Maintain accurate and timely clinical documentation.
Efficiently manage your travel time and daily schedule.
Schedule: Monday to Friday, 8:00 AM – 5:00 PM. No on-call or weekend requirements!
Qualifications Core RequirementsAssociate degree or higher in Physical Therapy.
1+ years of professional experience in Home Health Care.
Proficiency with Electronic Medical Record (EMR) systems (e.g., Practice Partner).
Valid Driver’s License and a Physical Therapy Assistant license in Pennsylvania.
