Position Summary
We are seeking a Licensed Speech-Language Pathologist (SLP) to provide skilled speech therapy services to individuals in their home and community environments. The ideal candidate has experience in home health and a background working with individuals with IDD, acquired neurological conditions, or complex communication needs. This role involves evaluating and treating speech, language, cognitive-communication, voice, fluency, and swallowing disorders, while collaborating closely with an interdisciplinary team to promote each individual's functional communication and quality of life.
Key Responsibilities
- Conduct comprehensive speech-language pathology evaluations including assessments of speech, language, cognitive-communication, voice, fluency, and swallowing function in the home setting
- Develop individualized plans of care in collaboration with the patient, family/caregivers, physician, and interdisciplinary team
- Provide skilled SLP interventions including articulation therapy, language intervention, AAC (augmentative and alternative communication) training, dysphagia management, and cognitive-communication rehabilitation
- Perform clinical swallowing evaluations and develop diet texture/liquid consistency recommendations in accordance with IDDSI guidelines
- Educate patients, family members, caregivers, and DSPs on communication strategies, feeding techniques, AAC device use, and home practice programs
- Collaborate with occupational therapists, physical therapists, nurses, behavioral specialists, and DSPs to ensure coordinated, person-centered care
- Complete all clinical documentation including evaluations, visit notes, progress reports, and discharge summaries accurately and within required timeframes
- Ensure compliance with Pennsylvania Title 28 (Chapters 601/611) home health regulations and applicable waiver program standards
- Participate in ISP meetings, care conferences, and QAPI initiatives as needed
Required Qualifications
- Current, unrestricted Pennsylvania Speech-Language Pathologist license in good standing
- Master's degree in Speech-Language Pathology from an accredited program
- ASHA Certificate of Clinical Competence (CCC-SLP) required or eligible
- Minimum 1–2 years of experience in home health or community-based speech-language pathology
- Valid Pennsylvania driver's license and reliable personal transportation
- Current CPR/BLS certification
- Ability to pass Pennsylvania criminal background check, child abuse clearance, and FBI fingerprinting
- Strong clinical documentation skills and knowledge of Medicare/Medicaid home health coverage criteria
Preferred Qualifications
- Experience working with individuals with IDD, autism spectrum disorder (ASD), cerebral palsy, stroke, or TBI
- Familiarity with AAC devices and communication support strategies for non-verbal or minimally verbal individuals
- Experience with OLTL/Community HealthChoices or ODP waiver programs
- Experience with Axxess home health EHR platform (strongly preferred)
- Knowledge of Pennsylvania Chapter 6100 or 6400 residential habilitation framework
- Experience collaborating with DSPs and residential habilitation teams
