LPN Referral Coordinator (Fully Remote)

The Licensed Practical Nurse (LPN) Referral Coordinator is responsible for overseeing the clinical eligibility of incoming referrals and supporting the admissions process. This role requires strong clinical judgment, communication skills, and a proactive approach to referral management. The Referral Coordinator ensures that all referrals are assessed appropriately and facilitates timely and accurate admissions into Eden facilities. This embodies the Eden philosophy and promotes a culture of accountability and positivity.

Facilities supported include:

• Edenbrook North
• Edenbrook South

Work Schedule: Monday to Friday, 8 am to 5 pm

Essential Functions:

• Lead the daily operations of the Intake Department, ensuring smooth and efficient workflow.
• Review and assess clinical referral information to determine appropriateness for admission.
• Document all referral-related communications and follow-up activities accurately and promptly.
• Communicate effectively with interdisciplinary teams regarding referral status and clinical needs.
• Collect and record all necessary data using approved forms and tools.
• Respond promptly and professionally to external referral sources via phone and email.
• Support the implementation of new technologies and systems to improve intake efficiency.
• Maintain accurate reporting systems to track intake performance and progress toward agency goals.
• Ensure compliance with all state licensure requirements and continuing education standards.
• Triage incoming calls using critical thinking and available patient records to assess needs.

Qualifications:

• Must have an active LPN license in Pennsylvania.
• Minimum of 2 years of Skilled nursing experience in Referral.
• Strong clinical assessment skills related to skilled nursing eligibility.
• Proficiency in Point Click Care
• Excellent interpersonal, organizational, and communication skills.
• Demonstrated emotional intelligence, sound judgment, and ability to foster positive team relationships.
