Pediatric Home Health Licensed Practical Nurse- Part-Time
About the LPN Position
We are looking for a reliable LPN (Licensed Practical Nurse) to care for individuals. You will work under the supervision of a registered nurse or doctor.
LPN professionals must be first and foremost qualified and compassionate. The ideal candidate will be experienced in providing different basic care needs to those who require it, while remaining patient, calm, and kind. It is a demanding job but also very rewarding.
LPN Responsibilities
Record a patient's medical history accurately
Take and record measurements of blood pressure, temperature, heart rate, etc.
Observe patients under treatment to identify progress, side-effects of medications, etc.
Monitor patients' condition including fluid intake and output and compose patient charts
Assist ill patients in daily basic activities such as eating, bathing, etc.
Administer injections, prescribed medications, enemas, etc., clean and dress wounds and help with other basic medical care tasks
Collect samples for lab testing as assigned
Provide emotional and psychological support when needed
Communicate with patients' family or friends to give advice, comfort and release instructions
LPN Requirements
Proven experience as LPN or similar role
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
Familiarity with hospital health, safety and sanitation standards and procedures
Understanding of confidentiality obligations and nursing best practices
Excellent knowledge of medical and hospital terminology
Solid knowledge of MS Office and data entry
Perfect physical condition and stamina
Excellent communication and interpersonal skills
Compassionate and able to handle stress
- Successful completion of LPN license program is a must
Reports to: Director of Nursing
This role involves assessing, planning, implementing, and evaluating total patient care while coordinating patient and family teaching.
Essential Duties and Responsibilities:
- Deliver professional nursing care to assigned patients.
- Assess patient's physical and emotional status, competency, home environment, safety factors, and family support.
- Provide information on patient rights, reimbursement procedures, and patient/family rights.
- Evaluate patient status, review and implement care plans, and communicate with attending physicians and clinical management.
- Perform nursing procedures and techniques as ordered by the physician, including administering medications and intravenous solutions.
- Supervise Home Health Aides as needed.
- Assist patients with activities of daily living and support their efforts towards self-sufficiency and independence.
- Maintain patient clinical records and reports.
- Ensure quality care is provided by Home Health Aides.
- Participate in on-call rotation as required.
- Perform other duties as assigned by management.
Supervisory Responsibilities:
- Oversee patient care, including direction, coordination, and evaluation of care and staff.
- Responsibilities include selection and training of employees, work planning, performance appraisal, and addressing complaints.
Qualifications:
- Active LPN license in good standing in Pennsylvania.
- Graduate of an accredited school of nursing.
- At least one year of professional nursing experience preferred but not required.
Skills and Abilities:
- Strong nursing, observation, communication, and supervision skills.
- Knowledge of home health care and government regulations.
- Good physical and mental health.
- Caring attitude, empathy, tact, and patience.
- Ability to solve practical problems and interpret various instructions.
Physical Demands:
- Requires moderate physical exertion including driving to patient homes, assisting in transfer activities, and providing support in ambulation therapy.
- Regularly required to use hands, talk, and hear; frequently required to stand, walk, reach, stoop, kneel, crouch, or crawl.
- Must occasionally lift and/or move over 100 pounds.
- Vision requirements include close vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
