Case Management Coordinator
Job Responsibilities:
- Establish trusting relationship with members.
- Make follow up calls and community visits to ensure member is connected to community services.
- Meet member in medical setting to ensure member has comprehensive and coordinated care.
- Assist member with completion of applications for medical or social benefits.
- Document all member encounters in documentation platform.
- Maintain excellent working relationships with members, families, providers and CBOs.
- Knowledge of community resources
Skills:
- As a priority, interactions will promote, maintain, and improve the health of members and their families.
- This includes to provide social support and advocate for individuals and community health needs.

Education/Experience:
- High School Diploma or GED completed.
- Bilingual – English and Spanish
- Lived experience in the community served.
- Ability to become a certified CHW.
