We’re currently seeking a dedicated LPN to serve as the Clinical Care Coordinator for our Personal Care community at Luther Acres, a Luthercare community in Lititz, PA. The position is a full-time, exempt position.

WHAT YOU DO:

As the Clinical Care Coordinator, you’re an integral part of ensuring residents in our Personal Care community receive effective and compassionate care to support their independence and overall health and well-being. You’ll do this in several ways, including:

  1. Supervise an assigned resident personal care area and assign work as appropriate. Provide instruction and direction in the methods and techniques of nursing and resident care. Ensure appropriate staffing coverage. Perform those functions required to manage staff including hiring, problem resolution, evaluations, and disciplinary action.
  2. Develop and present orientation and training programs to promote necessary skills of providers and ensure appropriate competencies are maintained. Observe staff performance in completing daily tasks and provide direction in improving the way work is completed.
  3. Assist with the prescreening of residents prior to admission and/or prior to return from hospitalization or other health care provider to determine appropriateness for personal care. Assist with the admission process.
  4. Demonstrate and coordinate accepted standards of performance by working as a Personal Care LPN and providing hands-on assistance to team members.
  5. Perform daily rounds to review documentation. Report changes in resident condition to the Personal Care Manager/Administrator. Complete all necessary documentation. Provide communication to family members regarding important issues and updates. Ensure labs are completed as ordered and addressed.
  6. Assist the Personal Care Manager/Administrator with the inspection process in accordance with state regulations and the standards of Luthercare.
  7. Maintain current knowledge of billing, coding, and insurance forms used in Personal Care.
  8. Assist with facility oversight in the absence of the Personal Care Manager/Administrator or at designated times.
  9. Provide communication with physicians regarding resident issues, orders, and treatments.
  10. Coordinate transportation with resident family members for appointments & provide Documentation of Medical Evaluation (DME).
  11. Complete Resident Assessment Support Plans (RASP) for residents in a timely manner, and other documentation required by the Department of Human Services (DHS). Contact family members to review RASP after completion.
  12. Complete assessments on residents, including admission, discharge, and level of living.
  13. Complete rounds with physicians, as needed. Provide information/updates to nursing staff, supervisors, physicians, and other disciplines as appropriate to coordinate continuity of care. Attend clinical meetings as assigned. Assist with orders on units.
  14. May assist with daily care of residents, as needed, to include duties such as administering medications, providing treatments as ordered by physician, and routine tasks such as BP, weights, intake/output, answering call bells, hands-on care.
  15. Adhere to all Personal Care standards, federal/state regulations and follow Luthercare’s infection control, fall prevention, general safety protocols and quality assurance and improvement standards.

EDUCATION & KNOWLEDGE REQUIREMENTS:

  1. High school diploma or equivalent
  2. Individuals in this position must have obtained age twenty-one (21) prior to the first day of employment or transfer into the position.
  3. Current & valid PA licensure as a Licensed Practical Nurse (LPN)
  4. Current Cardiopulmonary Resuscitation (CPR) certification and First Aid Training

ESSENTIAL JOB-RELATED EXPERIENCE/ABILITIES:

  1. Ability to read, write, and speak the English language fluently.
  2. Ability to reposition and turn residents and assist with transportation of residents within the facility.
  3. Ability to lift up to fifty (50) pounds occasionally and to perform the functions necessary to push a wheelchair.
  4. Excellent written and verbal communication skills and the ability to train end users.
  5. Proficient in the use of Microsoft Products such as Word, Excel and Power Point.
  6. Must be able to work with minimal supervision.
  7. Demonstrated ability to maintain a therapeutic attitude toward and effective relationship with the physically ill or infirm residents.
  8. Fit testing as needed per Luthercare’s Respiratory Program

EXPOSURE TO HAZARDOUS CONDITIONS:

  1. Exposure to blood and body fluids is seldom.
  2. Exposure to hazardous gas/chemical fumes is occasional.

PREFERRED JOB-RELATED EXPERIENCE/ABILITIES:

  1. Department of Human Services (DHS) Medication Assistance Training completed or willingness to do so.
  2. One (1) year experience with various software applications, basic computer skills, and use of electronic health records
  3. Personal Care Home Administrator License
