Qualifications

• Current unrestricted medical license in Pennsylvania
• Unrestricted DEA license for Pennsylvania
• Must be eligible to participate in Medicare
• DOT FMCSA certificate - active or willing to get during credentialing
• Medical degree (MD) or Doctor of Osteopathy (DO) degree from accredited institution
