Licensed Practical Nurse (LPN) - Home Health
Location: Pennsylvania (PA) Job Type: Full-Time / Part-Time / PRN Reports To: RN Case Manager or Clinical Supervisor
Job Summary:
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our Haddon Home Care team. The LPN will provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with the patient's individualized care plan. This position ensures patients receive high-quality, compassionate care to support recovery and maintain their highest level of independence.
Position Details:
- Schedule: Primarily Monday-Friday, 8 AM-4 PM, with occasional evening/weekend needs
- Pay: $30/hr
- Care: No bedside care; only light hands-on duties such as taking vital signs
- Work Style: Primarily remote/in the field, with occasional visits to our office in Jenkintown
As we establish our presence in the five counties (Bucks, Chester, Delaware, Montgomery, & Philadelphia), the role will start off part-time and grow with our client base.
Responsibilities include:
- Overseeing care provided by Direct Care Workers
- Conduct initial intake meetings to set up care for new clients
- Conducting DCW skills checks at orientation and annually
- Completing 30-day check-in calls with clients/families to ensure care plans are appropriate and satisfaction remains high
- Performing 60-day in-person home visits to confirm care plan compliance and client satisfaction

Qualifications:
- Valid LPN license in the state of Pennsylvania.
- Minimum of 2 years of clinical experience; home health experience preferred.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and communication skills.
- Experience with electronic medical records (EMR) is a plus.
Physical Requirements:
- Ability to travel to patient homes and work in various home environments.
- Able to lift/move patients, equipment, and supplies up to 50 lbs.
- Frequent standing, walking, bending, and reaching.
Work Schedule:
- Flexible shifts: Day / Evening / Weekend availability based on patient needs.
