Registered Nurse (RN) – Hospital at Home Program

Inpatient-Level Care Delivered in the Home

Program Overview

The Hospital at Home Program delivers acute, inpatient-level care to patients in the comfort of their homes. This program primarily serves general medical-surgical (MS) patients who meet eligibility criteria and are appropriate for home-based hospitalization.

Patients admitted to the program receive:

• 24/7 virtual nursing support
• Two in-home field nursing visits per day
• Diagnostic and ancillary services delivered in the home (labs, imaging, therapies, etc.)

This innovative care model combines bedside clinical expertise with virtual care technology to ensure safe, high-quality inpatient services outside of the traditional hospital setting.

Position Summary

We are seeking experienced Registered Nurses to support the launch of our Hospital at Home program (launch date: April 14). RNs will function across three key roles and must be flexible to work within each area as needed.

The ideal candidate has strong inpatient medical-surgical experience, is comfortable working independently in the home setting, and is proficient in electronic documentation systems (EPIC preferred).

Key Roles & Responsibilities

1. Admission/Enrollment RN (Brick-and-Mortar Setting)

• Identify and enroll eligible patients into the Hospital at Home program
• Coordinate transition from hospital unit to home setting
• Complete required documentation in EPIC
• Collaborate with interdisciplinary teams to ensure safe transition

2. Field RN (In-Home Care)

• Provide two in-home visits per day per assigned patient
• Deliver inpatient-level nursing care in the home
• Perform clinical procedures including:
  • IV insertion
  • Lab draws
  • Medication administration
• Monitor low-acuity medical-surgical patients
• Conduct patient assessments and ongoing evaluation
• Escalate concerns appropriately through virtual team

3. Virtual RN (Remote Monitoring & Support)

• Provide 24/7 virtual nursing oversight
• Conduct remote assessments and patient monitoring
• Support field nurses in clinical decision-making
• Coordinate care and respond to patient needs in real time

Schedule

• 12-hour shifts
• 7 days per week coverage
• Every other weekend (EOW) requirement

Shift Options:

• 7:00 AM – 7:30 PM (Field & Virtual blended role)
• 9:00 AM – 9:30 PM (Field Nurse)

Qualifications

Required:

• Active RN license
• BSN
• EOW
• Recent inpatient medical-surgical experience
• Experience with IV placement and lab draws
• Comfort caring for low-acuity inpatient population
• Strong documentation skills (EPIC proficiency preferred)
• Ability to work independently in home setting
• Flexibility to function across multiple program roles

Preferred:

• Experience in transitional care, home health, or care coordination
• Comfort with telehealth/virtual platforms
