Position Summary
We are seeking a credentialed Registered Dietitian (RD) to provide medical nutrition therapy and dietary consultation services to individuals in their home and community settings. The ideal candidate has experience working with medically complex patients and/or individuals with intellectual and developmental disabilities (IDD) in a home health or residential habilitation environment. This role involves conducting nutritional assessments, developing individualized nutrition care plans, addressing dysphagia-related dietary needs in collaboration with the SLP, and educating individuals, caregivers, and DSP staff on nutrition-related health promotion and disease management. The RD at QHCG plays a vital role in supporting holistic, person-centered care across all service lines.
Key Responsibilities
- Conduct comprehensive nutritional assessments in the home setting, including dietary history, anthropometric measurements, laboratory value review, and functional eating evaluations
- Develop and implement individualized Medical Nutrition Therapy (MNT) plans addressing chronic disease management, weight management, malnutrition, enteral nutrition, food allergies/intolerances, and other nutrition-related conditions
- Collaborate with the Speech-Language Pathologist to ensure dietary texture and liquid consistency recommendations align with dysphagia management protocols and IDDSI guidelines
- Partner with nursing staff, physicians, and the interdisciplinary team to integrate nutrition care into the overall plan of care for each individual
- Provide nutrition counseling and education to individuals, family members, caregivers, and DSPs on therapeutic diets, meal planning, food safety, hydration, and healthy eating strategies
- Monitor and reassess nutritional status over time, adjusting care plans based on changes in clinical condition, lab values, or individual goals
- Support residential habilitation homes with menu planning guidance, dietary policy compliance, and staff education on meeting the nutritional needs of individuals with IDD
- Maintain accurate and timely clinical documentation including assessments, care plans, progress notes, and discharge summaries in compliance with agency and payer requirements
- Ensure all services are delivered in compliance with Pennsylvania Title 28 home health regulations and applicable waiver program standards
- Participate in ISP meetings, care conferences, and QAPI initiatives as needed
Required Qualifications
- Current registration as a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) with the Commission on Dietetic Registration (CDR)
- Current Pennsylvania dietitian-nutritionist licensure in good standing
- Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program; completion of an accredited dietetic internship
- Minimum 1–2 years of clinical nutrition experience in a home health, long-term care, residential, or community-based setting
- Strong knowledge of medical nutrition therapy for chronic conditions including diabetes, renal disease, cardiovascular disease, obesity, and malnutrition
- Valid Pennsylvania driver's license and reliable personal transportation
- Ability to pass Pennsylvania criminal background check, child abuse clearance, and FBI fingerprinting
- Current CPR/BLS certification
- Strong clinical documentation and patient education skills
Preferred Qualifications
- Experience providing nutrition services to individuals with IDD, autism spectrum disorder (ASD), cerebral palsy, or other developmental disabilities
- Familiarity with enteral nutrition management and tube feeding protocols in the home setting
- Experience collaborating with Speech-Language Pathologists on dysphagia and texture-modified diet management
- Knowledge of Pennsylvania ODP or OLTL/Community HealthChoices waiver programs
- Experience with Axxess home health EHR platform (a plus)
- Familiarity with Therap or similar IDD documentation platforms
- Certified Diabetes Care and Education Specialist (CDCES) or other specialty credential (a plus)

This position operates primarily in patient homes, residential habilitation sites, and community settings throughout Delaware County and surrounding areas.
