Home Health and Hospice RN (12pm-8 pm)

Department: Home Health & Hospice.
Shift: Monday–Friday, 12PM–8PM.

Are you a compassionate and skilled nurse looking for a dynamic role that combines both Home Health and Hospice care? Join the dedicated team at the VNA of Hanover & Spring Grove as our 2nd Shift RN, where you'll make a meaningful impact on patients' lives every day.

This hybrid RN role offers the best of both worlds. From 12PM–4PM, you’ll manage a small Home Health caseload. From 4PM–8PM, you’ll transition into Hospice care, handling consents, IVs, and emergent needs. You’ll work independently while remaining an integral part of an interdisciplinary team, delivering timely, high-quality, and compassionate care in alignment with agency policies and Medicare/Pennsylvania guidelines.

Key Responsibilities:

• Conduct assessments and develop personalized care plans.
• Initiate after-hours Hospice consents and IV administration.
• Provide patient and family education using teach-back techniques.
• Monitor condition changes and coordinate care with physicians and teams.
• Document care accurately and in a timely manner in the EHR.
• Participate in care planning meetings and support quality initiatives.
• Supervise aides and provide mentorship to peers.

What We’re Looking For:

• RN license in good standing (PA).
• Graduate of an accredited RN program.
• At least one year of nursing experience (Home Health or Hospice preferred).
• CPR certification.
• Strong critical thinking, documentation, and communication skills.
• Reliable transportation, valid PA driver’s license, and current auto insurance.

Physical/Work Requirements:

• Able to lift up to 50 lbs and work in various patient home environments.
