Responsibilities

• Assess and treat patients with PT plans based on individual goals.

• Develop and implement evidence-based treatment plans with measurable milestones.

• Monitor patient progress and adjust therapies to optimize outcomes.

• Maintain licensure, adhere to professional standards, and ensure patient safety.

• Document patient encounters accurately in electronic health records.

• Collaborate with a multidisciplinary team to align on care plans.

• Contribute to monthly targets and pipelines by documenting potential patient referrals.

• Educate patients and caregivers on exercises and home programs.

• Identify and communicate barriers to progress and adjust plans accordingly.

• Participate in continuous learning and professional development activities.

• Support on-site operations, ensuring efficient patient flow and wait times.

Experience/Qualifications

• Licensed Physical Therapist with active state license and clean practice record.

• Develop and implement evidence-based PT plans with measurable milestones.

• Regular progress monitoring and therapy adjustments to optimize outcomes.

• Meet monthly targets and contribute to pipeline by documenting potential referrals.

• On-site patient flow optimization to minimize wait times and maximize throughput.

• Accurate documentation of patient encounters in electronic health records (EHR).

• Collaborate with multidisciplinary team to align on care plans and referrals.

• Identify barriers to progress and adjust plans accordingly.

• Educate patients and caregivers on exercises, home programs, and safety.

• Commitment to ongoing professional development and participate in training.
