Licensed Practical Nurse (LPN)

ESSENTIAL FUNCTIONS:
· Provide guidance and direction to DSPs and RHMs related to medical concerns and needs
· Act as a hospital, Skilled Nursing Facilities (SNF), and overall health care liaison
· Coordinate hospitalization discharges, including completion of in-person assessments
· Support individuals in achieving health-related goals and advocate for their medical needs
· Complete Health Risk Screening Tool (HRST) clinical reviews
· Participate as a member of the Individual Service Plan (ISP) team as needed
· Participate in the completion of the intake assessment on all new admissions
· Ensure compliance with regulations set forth by the state, federal, ODP, and OLTL
MINIMUM QUALIFICATION:
· Graduate of an accredited practical nursing program, required
· Current, active license as an LPN in the state of Pennsylvania, required
· Minimum of one year of experience working with individuals with developmental disabilities (I/DD) and/or Traumatic Brain Injury (TBI), preferred
REQUIRED SKILLS:
· Strong clinical assessment and decision-making skills
· Knowledge of medical terminology, pharmacology, and standard nursing practices
· Ability to communicate effectively, both verbally and in writing, with individuals served, staff, families, healthcare providers, and service provider team members
· Ability to keep meticulous documentation, maintain confidentiality, and adhere to HIPAA regulations
· Proficiency in electronic health records (EHR) and understanding basic computer skills, including Microsoft applications
· Familiarity with Therap system ( preferred )
· Knowledge of 6400 Regulations ( preferred )
OTHER JOB REQUIREMENTS:
· Current valid Pennsylvania Driver’s License, required
· Have access to reliable transportation, required
· CPR/FA/AED Certification, within 6 months of hire
· Medication Administration Trainer Certification, within 6 months of hire
· Flexibility to work evenings, weekends, or holidays as needed
WORK ENVIRONMENT & PHYSICAL DEMANDS/REQUIREMENTS:
· Occasionally travel locally with the individual
· Regularly travel to residential home locations
· Regularly work non-traditional hours, including participation in incident management reporting duties during nights, weekends, and holidays, as assigned
· Occasionally fill in for a direct support professional shift, including during non-traditional hours, as needed
· Regularly work in a climate-controlled home or office environment or another approved location in the community
· Frequent standing, walking, bending, reaching, and occasional assisting with transfer
· Possible exposure to communicable disease and bodily fluids
· Use of standard nursing equipment and personal protective equipment (PPE) required
· Ability to see and interact with text and graphical information on a screen
· Ability to receive and respond to phone calls
· Ability to use office equipment
· Ability to lift 50 pounds
