Position Overview

As a Case Manager at Pristine Home Care, you will play a vital role in supporting our clients, ensuring smooth case management, and fostering exceptional standards in staff and patient care. You will oversee a caseload of adult clients in unskilled home care, developing individualized care plans, providing ongoing case management, and serving as an advocate for client needs. This vital role involves conducting home visits, collaborating with families and community partners, and ensuring that each client receives appropriate medical, social, and community support. The Case Manager must have a passion for making a positive impact on the lives of others.

Key Responsibilities

• Conduct initial client assessments and develop personalized care/service plans.
• Provide crisis intervention and management for clients as needed.
• Coordinate access to healthcare services and benefits, including Medicare.
• Travel to client homes for assessments, home safety checks, and documentation.
• Address addiction, substance abuse, and refer to counseling or appropriate services.
• Collaborate with community resources and organizations for comprehensive support.
• Monitor client progress; adjust care plans to optimize outcomes.
• Document all case management activities promptly and accurately.
• Advocate for clients’ needs and connect them to necessary resources and systems.
• Supervise/mentor junior staff when appropriate.

Required Skills, Qualification and Experience

• Bachelor’s degree preferred (social work, human services, or related field).
• Prior experience in case management, social services, or related healthcare coordination.
• Knowledge of social work practices and regulatory requirements.
• Experience with crisis management and Section 8 housing programs.
• Excellent verbal and written communication skills.
• Strong organizational and time management skills.
• Professionalism, discretion, and the ability to maintain confidentiality.
• Experience with workforce development and job placement (a plus).
• Ability to manage multiple priorities in a fast-paced environment.
• Valid driver’s license and reliable transportation for home visits.
• CNA certification (preferred, not required).
• Comfort using scheduling and documentation software (training provided if needed).
• The role requires proficiency in using a personal computer, calculator, multi-line telephone system, cell phones, and other standard office equipment.

Compensation & Benefits

• Salary: $18 - $20 (depending on experience and qualifications)

Work Environment & Schedule

• Primarily in the field/on the road, with visits to clients’ homes and community locations.
• Flexible schedule: 8 hour shifts, with potential for on-call hours.
