Responsibilities
- Perform initial evaluations and ongoing assessments of patients in home-based settings.
- Develop and implement individualized physical therapy plans focused on mobility, strength, balance, gait, and functional independence.
- Provide therapeutic interventions and patient/caregiver education to support safe, effective home exercise programs.
- Monitor progress, document goals and outcomes, and adjust plans as needed in the patient’s health records.
- Collaborate with an interdisciplinary team to coordinate care and ensure smooth transition across services.
- Maintain patient safety, infection control, and equipment management within home environments.

Requirements
- Active physical therapy license in Pennsylvania or eligibility to obtain licensure.
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program.
- 1 year of clinical PT experience is preferred; home health or outpatient experience is a plus.
- CPR/BLS certification preferred or required by local policy.
- Ability to work effectively in a home healthcare setting and travel within the service area.
- Strong communication, documentation, and time-management skills; ability to work independently and as part of a team.
