Physical Therapist - Home Health | Full-Time PT Position in Lancaster

Physical Therapist Responsibilities
- Conduct comprehensive patient evaluations and assessments to determine individualized physical therapy needs and functional limitations
- Develop and implement evidence-based treatment plans focused on improving mobility, strength, balance, and functional independence
- Provide skilled physical therapy interventions including therapeutic exercises, manual therapy, gait training, and pain management techniques
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to deliver coordinated, multidisciplinary care
- Educate patients and family members on home exercise programs, fall prevention strategies, and adaptive equipment usage
- Monitor and document patient progress, outcomes, and response to treatment interventions
- Modify treatment plans based on ongoing assessments and patient goals
- Ensure compliance with home health regulations, documentation requirements, and quality standards
- Participate in care coordination and discharge planning activities
Physical Therapist Qualifications & Requirements
- Current, unrestricted Physical Therapist license in the state
- Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program
- CPR certification required
- Strong clinical assessment and critical thinking skills
- Excellent verbal and written communication abilities
- Compassionate, patient-focused approach to rehabilitation care
- Outstanding organizational and time management skills
- Ability to work independently in home settings with minimal supervision
- Valid driver's license and reliable transportation for home visits
- Home health physical therapy experience preferred but not required
- New graduates and experienced PTs welcome to apply
