Essential Functions of the Case Manager:

  • Provide direct, in-person services to youth and families according to established program time frames, protocols, and attendance support goals. A minimum of 50% of working hours will be spent providing direct services to identified clients.
  • Deliver individual, parent, and family case management services in client homes, schools, agency offices, and other appropriate locations, in alignment with treatment plans and Attendance Support Program requirements.
  • Engage, support, and retain assigned youth and families throughout the service period by building rapport, fostering motivation, and implementing strategies that address attendance barriers and promote academic engagement.
  • Work collaboratively with school partners and community agencies to gather attendance data, coordinate supports, participate in attendance meetings, and connect families with external resources as needed.
  • Develop, monitor, and update individualized service plans including goals related to attendance improvement, coping skills, family communication, and school connectedness.
  • Complete and submit all required administrative and clinical documentation within established deadlines and in accordance with agency and program standards.
  • Achieve identified treatment and program outcomes with assigned youth and families, including measurable progress toward attendance improvement and reduction of barriers.
  • Maintain accurate scheduling and caseload management, ensuring required number of face-to-face contacts, follow-up calls, outreach attempts, and family meetings.
  • Participate in supervision, team meetings, and required trainings, applying feedback to ensure high-quality service delivery and compliance with program standards.
  • Transport self to client homes, schools, and community locations. This includes the ability to safely enter and exit various home environments, climb stairs when necessary, and navigate typical community settings.
  • Maintain professional boundaries, confidentiality, and safety procedures during all service interactions, including home visits and community-based work.

Minimum Qualifications of the Case Manager :

  • Associate's Degree in a human services field or equivalent lived experience demonstrating the ability to support youth and families effectively.
  • Demonstrates maturity, sound judgment, and the ability to serve as a positive role model for youth and families.
  • Possesses a strong understanding of adolescent development, behavior, and family systems.
  • Exhibits strong organizational skills and the ability to manage multiple tasks, timelines, and responsibilities.
  • Able to work creatively and flexibly with diverse populations, adapting approaches to meet the needs of individuals, families, and community partners.
  • Must hold and maintain a valid driver's license, vehicle insurance, and current vehicle inspection for any vehicle used during work and/or client transportation.
  • Must achieve and maintain a passing annual motor vehicle record clearance to operate agency vehicles.
  • Proficient with computers and software, including Microsoft Windows; familiarity with external databases and Google Suite is preferred.
  • Must have no convictions for prohibited criminal offenses or child abuse violations, in accordance with state and federal regulations.

Preferred Qualifications of the Case Manager:

  • A year of CASSP experience.

Physical Requirements for the Case Manager:

  • Sedentary work that primarily involves sitting/standing. X Occasionally.
  • Light work that includes moving objects up to 20 pounds. X Occasionally.
  • Navigating stairs and tight spaces (accessing older buildings possibly through fire escapes, narrow staircases, and tight walkways). X Occasionally.
  • Driving to include long distance (2-6 hours in one day). X Occasionally.
