Extended Family Care, a well established Home Care Agency in Lancaster, PA has an Immediate Need for a full-time Registered Nurse Supervisor (RN)

$3000.00 SIGN ON BONUS

SUMMARY

Our RNs provide direct patient care; teach and supervise others in aspects of patient/family care, evaluate patient/family needs, plans patient care and refer patients for other services as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

• Obtains patient's medical history, particularly as it relates to the patient's present condition, from the patient or family member.
• Performs physical assessments to identify patient needs.
• Evaluates the patient's family dynamics, home situation and environment.
• Evaluates the patient's condition to determine what type and frequency of services are required and obtains physician's orders for same.
• Establishes, maintains, and/or revises the patient's plan of care.
• Evaluates the effectiveness of the plan and makes revisions based on patient's changing needs.
• Informs physician of changes in the patient's condition, plan of care and/or frequency and type of services needed.
• Consults and coordinates care with other active caregivers.

JOB REQUIREMENTS

• Graduate of an approved school of nursing.
• Previous home care experience preferred.
• Must possess a current license to practice in the State as an RN.
• Must be able to read and interpret documents and manuals.
• Ability to write routine reports and correspondence.
• Ability to speak effectively before patients and their families.
• Computer literate.
• Must have reliable transportation – individual will make multiple supervisory home visits each day for the branch.
