Responsibilities
- Review resident medical records to determine appropriate level of care and support utilization management decisions.
- Coordinate with the interdisciplinary care team to develop and implement care plans aligned with patient needs and payer requirements.
- Conduct chart reviews to identify gaps, support discharge planning, and monitor for timely care transitions.
- Obtain prior authorizations and communicate with payers and providers as needed.
- Document all utilization management activities in the charting system in compliance with HIPAA and facility policies.

Requirements
- Active RN license in Pennsylvania (or eligibility for licensure in PA)
- Experience in utilization management, care coordination, or related field preferred
- Strong chart review, documentation, and communication skills
- Ability to work independently within a skilled nursing/long-term care setting
- Availability to work the scheduled shift and 40 hours per week
