Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.
We are seeking a qualified Manager for our Delaware County Program. The Manager establishes, administers, and directs the Program of Forensic Assertive Community Treatment (FACT), a self-contained clinical team which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified clients with severe and persistent mental illnesses; supervises and evaluates the multidisciplinary team in conjunction with appropriate psychiatric support to ensure courteous, helpful and respectful services to program clients; and functions as a practicing clinician on the team.
This position is also responsible for but not limited to the following duties
- Direct the day-to-day clinical operations of the FACT team including scheduling staff work hours to assure appropriate coverage for day, evening, weekend, and holiday shifts and on-call hours; lead the daily organizational staff meetings and treatment planning meetings; continuously evaluate the status of clients and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs.
- Manage and coordinate, for each client, the client-centered comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
- Participate in staff recruitment, interviewing, hiring, work assignments, and orientation and performance supervision according to work rules, regulations, and policies and collective bargaining agreements; develop and implement staff orientation and training; and manage student training.
Develop and administer the FACT program budget including line items to purchase furniture, equipment, communications devices (e.g., telephones, cellular phones, beepers) and supplies and to cover travel and transportation (e.g., purchase or lease program cars, or cover mileage and insurance for use of personal staff vehicles or both).
Qualifications:
- Master’s degree in nursing, social work, psychiatric rehabilitation, psychology, or is a psychiatrist.
- Licensed or certified according to the laws of the state in which the program is operating.
- Strong clinical skills and experience providing treatment to and establishing supportive trusting relationships with persons with severe and persistent mental illnesses and respect client rights and personal preferences in treatment.
- Supervisory and program management experience is desirable.
Must be knowledgeable about the FACT model.
Scope:
- May require crisis intervention in emergency situations.
- Required to carry a beeper and be available to respond to emergency situations.
- May be required to attend training to become a certified agency investigator. May conduct investigations and participate in the agency rotation of on-call investigators. This requires carrying a beeper and responding to investigation hotline calls during on-call beeper coverage.
- Frequent talking and listening.
- Requires the ability to lift, carry, hold, kneel, reach, stack, stoop and bend, which may include direct intervention with clients.
- Must be available for local travel and possess a valid driver's license.
