We are seeking a dedicated and compassionate Full Time Home Health Physical Therapist (PT) to join our dynamic healthcare team. In this role, you will play a crucial part in enhancing the quality of life for our patients by providing tailored physical therapy services in the comfort of their homes.
Key Responsibilities:
- Conduct comprehensive assessments to determine the physical therapy needs of each patient.
- Develop personalized treatment plans aimed at improving mobility, strength, and overall physical function.
- Implement therapeutic exercises and techniques to address patient-specific goals.
- Educate patients and their families on home exercise programs and techniques to ensure continuity of care.
- Collaborate with other healthcare professionals to provide holistic and coordinated care.
- Document patient progress and modify treatment plans as necessary to achieve optimal outcomes.
Qualifications:
- Valid state licensure as a Physical Therapist.
- Proven experience in home health or a related healthcare setting is preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and manage a flexible schedule.
- Commitment to providing high-quality patient care.
