LPN - Licensed Practical Nurse

Join the Deer Meadow's Home Health team as a Licensed Practical Nurse (LPN) and provide top-tier home care services. Leverage your nursing skills to assess patient needs, follow care plans, and administer treatments. Collaborate with physicians and community health professionals to deliver comprehensive care.
Responsibilities:
-Implements plan of care initiated by the registered nurse.
-Administers medications and treatments as prescribed by the physician.
-Participates in on-call duties as defined by the on-call policy.
-Taking direction from clinical team and overseen by a registered nurse

Qualifications:
-Graduate of an accredited practical nurse or vocational nursing program.
-Licensed LPN in the state of PA
-Valid driver's license and reliable transportation
-Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
-CPR certification
