• Experience: Active RN license required; hospice, home health, or home care experience preferred
• Compensation: Competitive annual salary plus additional compensation for on-call coverage

Qualifications

• Active RN license in PA
• Hospice experience preferred; home health or related post-acute experience considered
• Strong clinical assessment and care planning skills
• Ability to manage a defined caseload independently
• Strong communication skills with patients, families, and interdisciplinary teams
• Willingness to participate in an on-call rotation

Responsibilities

• Manage and coordinate hospice care for patients within a defined Philadelphia territory
• Conduct comprehensive assessments and develop individualized care plans
• Collaborate with physicians and interdisciplinary team members to ensure quality care
• Educate patients and families regarding comfort-focused treatment and support
• Maintain accurate and compliant clinical documentation
• Participate in on-call rotation as scheduled
