Job Overview
We are seeking a compassionate and dedicated Social Worker (MSW) to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families in need, particularly those dealing with medical, emotional, and social challenges. This role requires a strong understanding of case management, patient care, and the ability to navigate complex healthcare systems. The Social Worker will work closely with clients to develop effective treatment plans and provide resources to enhance their quality of life. There are no nights, holidays or weekends or on call responsibilities associated with this job.
Duties
- Conduct assessments to identify client needs and develop individualized care plans for Hospice patients.
- Provide counseling and support to clients dealing with various life challenges, including mental health issues and developmental disabilities.
- Maintain accurate medical documentation and records in compliance with healthcare regulations.
- Collaborate with healthcare professionals, including physicians and nurses, to coordinate patient care.
- Facilitate access to community resources, services, and programs for clients.
- Advocate for clients' rights and needs within the healthcare system.
- Monitor client progress and adjust care plans as necessary.
- Provide education on dementia care, special education services, and other relevant topics.
Skills
- Proficient in medical documentation practices.
- Knowledge of ICD-10 coding for accurate diagnosis reporting.
- Strong case management skills with the ability to manage multiple cases effectively.
- Familiarity with working with individuals in Hospice care.
- Excellent communication skills, both verbal and written.
- Empathy and active listening skills to understand client needs deeply.
- Ability to work collaboratively within a multidisciplinary team.

Pay: $40,000.00 - $50,000.00 per year
