Job Description:
Sunbelt Staffing is looking for an experienced Home Health RN to join a top-tier healthcare team in Altoona, PA. This role involves providing direct patient care in a home setting, requiring strong assessment skills, OASIS documentation expertise, and proficiency in Homecare Homebase. The ideal candidate is self-sufficient, organized, and compassionate about delivering quality patient care.
Key Responsibilities:
- Conduct in-home patient assessments and develop individualized care plans
- Administer treatments, medications, and wound care as needed
- Complete OASIS documentation accurately and efficiently
- Provide patient and family education on home health care best practices
- Participate in on-call rotation as required
Qualifications:
- Active RN license in Pennsylvania
- Minimum of 2 years of recent home health experience
- Strong proficiency in OASIS and Homecare Homebase
- Current BLS certification (AHA)
- Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
