Description:
- We are seeking a dedicated professional to assist individuals in navigating the mental health care system.
- This full-time role is based in Philadelphia, PA, and requires a commitment to providing comprehensive support and advocacy for clients.
- The ideal candidate will have experience in human services and a deep understanding of community resources and crisis intervention.
Requirements:
- Conduct functional assessments to identify client needs and strengths.
- Develop and implement service plans for clients.
- Connect clients to clinical and community resources.
- Maintain case management records accurately.
- Provide crisis resolution services to minimize emergency interventions.
- Ensure 24-hour crisis response availability.
- Participate in interagency meetings and coordinate services.
Qualifications:
- Bachelor's Degree in a social science field.
- Two years of experience in human services with direct client contact.
- Knowledge of crisis intervention and community resources.
- Familiarity with behavioral health and social service systems.
- Bilingual (Spanish/English) is a plus.
- Strong decision-making and communication skills.
- Proficiency in Microsoft Office.
