Transitional Care Specialist
Are you looking to make a difference in the lives of others? Are you compassionate and caring? If so, Independent Living Services is looking for Transitional Care Specialist to provide priority care to adults with physical disabilities, our senior population and our Veterans in Lancaster County.
At Independent Living Services, we believe our first responsibility is to the individuals who count on us to provide quality care for them so that they are able to remain living safely and independently in their own home. We also believe that our employees are our most valuable resource and many of our staff has spent years working with our participants.
A Transitional Care Specialist is responsible for responding to priority care needs for new participants and/or provide fill-in coverage as needed.
MONDAY - FRIDAY (8AM - 4PM)

Our dedicated, compassionate Transitional Care Specialist provide:
- Personal Hygiene Care
- Dressing/Grooming
- Meal Preparation
- Light Housekeeping
- Companionship
Transitional Care Specialists must be at least 18 years of age and have a medical background (CNA, LPN, EMT) or 2 years of previous experience providing personal care is required. In order to be considered, an applicant must meet the criminal background guidelines set forth by the Office of Aging. Must have a valid driver's license as there will be a company vehicle available for your shifts.
