Full Time Evenings- 2:30pm-11:00pm| 64 hours/biweekly| Every other weekend and holiday rotation.
Rates starting at $31.00/hour, plus $2.00 shift differential for evenings and an additional $1.00 for weekends.

About the Role
At Frederick Living, the LPN provides skilled nursing care to residents while leading the neighborhood team. This role promotes Person-Centered Care, supervises daily resident care, and supports the professional growth of staff.
What You’ll Do
- Observe and report changes in resident condition to the Nurse Supervisor or physician/physician extender
- Communicate with Supervisor, ADON, DON, NHA, care providers, and family/POA as needed
- Use evidence-based practices to ensure safe, high-quality care delivery
- Assist in managing personnel through performance evaluation, Care Chats, coaching, counseling, education, and corrective action as needed
- Complete all required documentation accurately and in a timely manner
- Participate in resident transitions of care, care conferences, and care plan reviews
- Collaborate with Cedarwood staff to ensure optimal resident outcomes
- Comply with Infection Control practices
- Ensure hand-held communication equipment is maintained for effective call bell management and team communication
- Attend and complete all required in-services, training, and team meetings
- Abide by the organization’s code of conduct
- Perform other duties as assigned
What We’re Looking For
- Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania
- High school diploma or equivalent
- Prior experience in geriatric nursing preferred
- BLS Healthcare Provider certification
- Basic computer skills
- Strong communication skills, both verbal and written
- Ability to role model professional appearance and demeanor
- Demonstrates respect, integrity, compassion, and excellence
- Commitment to person-centered care initiatives
- Conflict resolution skills and ability to coach and support staff
- Flexible schedule and willingness to work extra hours as needed
- May be asked to assist in emergencies and resident evacuations
