SUMMARY OF POSITION:
The Heart Failure (HF) Nurse Navigator is responsible for coordinating and facilitating the care of patients diagnosed with heart failure across the healthcare continuum. The navigator functions as a central resource for patients, families, and the multidisciplinary care team to ensure evidence-based, coordinated, and patient-centered care.
Under the guidance of the East Region Performance Improvement Manager, the HF Nurse Navigator supports the Heart Failure Program through proactive patient identification, care coordination, education, and performance improvement initiatives aimed at improving clinical outcomes and reducing preventable utilization.
This role maintains a strong emphasis on patient and caregiver engagement, care plan comprehension, and transitions of care, which are key drivers of reduced readmissions and improved heart failure outcomes. The navigator collaborates closely with the HF Program Coordinator, Critical Care Unit Coordinator, physicians and advanced practice providers, care coordinators, social workers, clinic staff, Community Paramedicine teams, and Clinical RX-2-Go Pharmacists to ensure seamless care delivery across inpatient, outpatient, and community settings.
MINIMUM QUALIFICATION(S):

Bachelor's degree in Nursing

Three (3) years of nursing experience

Currently licensed to practice as a registered nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact

Certified Professional in Patient Safety (CPPS) certification within one (1) year of hire

AHA Advanced Cardiovascular Life Support (ACLS) - required prior to the end of the orientation period

AHA Basic Life Support (BLS) - required prior to the end of the orientation period

ED, Med Surg or ICU Experience
