Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments in patients' homes to determine functional limitations and rehabilitation needs
• Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall functional independence
• Deliver evidence-based physical therapy interventions including therapeutic exercises, manual therapy, gait training, and pain management techniques
• Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate comprehensive patient care
• Provide patient and caregiver education on home exercise programs, fall prevention strategies, and adaptive equipment use
• Document patient progress, treatment outcomes, and clinical notes in compliance with Medicare and insurance requirements
• Monitor and adjust rehabilitation plans based on patient response and progress toward established goals
• Maintain professional standards and adhere to home health regulations and quality assurance protocols

Required Qualifications

• Current, unrestricted Physical Therapist license in the state
• Graduate degree (DPT, MPT, or equivalent) from an accredited physical therapy program
• Valid driver's license and reliable transportation for home visits
• Strong clinical reasoning and assessment skills
• Excellent verbal and written communication abilities
• Compassionate, patient-centered approach to care delivery
• Exceptional organizational and time management skills
• Ability to work independently while maintaining professional accountability
• CPR certification (or willingness to obtain upon hire)

Preferred Qualifications

• Previous home health physical therapy experience
• Experience working with geriatric or medically complex populations
• Knowledge of OASIS documentation and Medicare home health guidelines
• Bilingual skills (Spanish/English) are a plus
