This role requires expertise in hospices and using Home Care Home Base (HCHB). The position is hybrid and involves managing patient care across Primary Cumberland/Perry County, with potential travel across other areas as needed.

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Address and reassess cost-efficient, appropriate levels of care
• Coordinate non-medical support to ensure treatment plan compliance
• Support care transitions and collaborate with multidisciplinary teams
• Document detailed records of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as a patient advocate, maintaining privacy and confidentiality

Requirements of the RN - Case Management

• RN License
• Valid Driver's License
• Valid Car Insurance
• Valid BLS CPR Certification
• Hospice and HCHB experience
• Associates or Bachelor's Degree in Nursing or related field
• Ability to analyze complex medical information
• Case Management certification
