Job Duties

• Provide compassionate, high-quality nursing care to residents.

• Administer medications and treatments as ordered.

• Monitor resident conditions and communicate changes to the RN and physician.

• Complete accurate and timely clinical documentation.

• Collaborate with interdisciplinary teams to ensure exceptional resident outcomes.

• Support admissions, discharges, and resident care planning.

• Maintain compliance with state, federal, and facility regulations.

Qualifications

• Current Licensed Practical Nurse (LPN) license in the applicable state.

• Current CPR certification (or ability to obtain).

• Ability to travel to multiple assigned facilities.

• Excellent clinical, communication, and organizational skills.

• Skilled nursing or long-term care experience is preferred, but motivated new graduates are encouraged to apply.
