Job Title: Physical Therapist (PT)- Home Care

Locations: Philadelphia, Delaware and Montgomery Counties

Join our team as a Physical Therapist (PT) and provide top-tier home care services. Leverage your therapy skills to assess patient needs, follow care plans, and administer treatments. Collaborate with physicians and community health professionals to deliver comprehensive care.

Responsibilities:
-Directs and provides physical therapy treatment in the home and assessing and evaluating home environment.
-Determining priority needs for physical therapy.
-Reporting to physician patient's reaction to treatment or changes in condition.
-Identifies patient and family/caregiver needs for other home health services and refers as necessary.
-Participates in discharge planning for patient.
-Supervises physical therapy assistants according to organization policy and state regulations.

Qualifications:
-Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
-Licensed to practice as physical therapist within the state, if applicable.
-Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
-Possesses and maintains current CPR certification.
-Valid driver's license and reliable transportation
