After Hours Registered Nurse (RN) - Hospice
Residential Home Health and Hospice is seeking a compassionate After Hours Registered Nurse (RN) to provide end-of-life care to patients in their homes. This role offers autonomy, meaningful patient relationships, and the opportunity to make a real impact during critical moments. You will be supported by a dedicated field education team and a collaborative leadership culture.
Responsibilities - Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team.
- Respond to after-hours and on-call patient needs by performing urgent assessments, managing symptoms and pain, and providing end-of-life support.
- Support and guide patients and families during evenings, nights, weekends, and crisis situations.
- Apply clinical judgment and critical thinking to coordinate care, respond to changing conditions, and maintain accurate EMR documentation.
- Travel to patient homes within the assigned territory.
Qualifications
- Active Registered Nurse (RN) license in the state of Pennsylvania.
- Minimum of two years of RN experience.
- Clinical expertise to deliver quality patient care; Hospice experience is preferred.
- Current driver’s license and ability to travel to patient locations.
