Position Summary
We are seeking a skilled and compassionate Behavioral Therapist to join our home health and community-based services team. The ideal candidate has experience supporting individuals with intellectual and developmental disabilities (IDD), behavioral health needs, or co-occurring conditions in residential habilitation or home settings. This role involves conducting behavioral assessments, developing and implementing individualized behavior support plans, and working collaboratively with individuals, families, caregivers, and interdisciplinary teams to promote positive outcomes and functional independence.
Key Responsibilities
- Conduct functional behavioral assessments (FBAs) and analyze behavioral data to identify patterns, triggers, and underlying causes of targeted behaviors
- Develop, implement, and monitor individualized Behavior Support Plans (BSPs) in compliance with Pennsylvania ODP/OLTL and applicable regulatory standards
- Provide direct behavioral intervention in the home and community setting using evidence-based approaches including ABA, positive behavior support (PBS), and trauma-informed care principles
- Train and coach Direct Support Professionals (DSPs), family members, and caregivers on behavior intervention strategies, de-escalation techniques, and plan implementation
- Collaborate with the interdisciplinary team including case managers, nursing staff, physical/occupational therapists, and residential hab staff to ensure integrated, person-centered care
- Monitor individual progress toward behavioral goals and adjust plans accordingly based on data and clinical judgment
- Document all assessments, session notes, progress reports, and plan updates accurately and in compliance with agency, waiver, and payer requirements
- Participate in ISP (Individual Support Plan) meetings, care conferences, and QAPI activities as needed
- Ensure all services are delivered in compliance with Title 55 (Chapters 6100/6400) and applicable OLTL/Community HealthChoices waiver requirements
- Maintain professional, ethical, and culturally competent practice standards at all times
Required Qualifications
- Bachelor's degree in Psychology, Social Work, Applied Behavior Analysis, Human Services, or a related field (Master's preferred)
- Minimum 1–2 years of experience providing behavioral support services in a home health, residential habilitation, or IDD setting
- Working knowledge of behavior analysis principles, positive behavior support, and person-centered planning
- Strong clinical documentation and data collection skills
- Valid Pennsylvania driver's license and reliable personal transportation
- Ability to pass Pennsylvania criminal background check, child abuse clearance, and FBI fingerprinting as required under applicable regulations
- Current CPR/BLS certification
- Strong communication and interpersonal skills; ability to work independently in home and community environments
Preferred Qualifications
- Board Certified Behavior Analyst (BCBA) credential or Registered Behavior Technician (RBT) certification
- Experience working within Pennsylvania ODP or OLTL waiver systems
- Familiarity with Therap or similar EHR/documentation platforms used in IDD settings
- Experience with Axxess home health EHR platform (a plus)
- Knowledge of Pennsylvania Chapter 6100 or 6400 regulatory framework
- Experience collaborating with residential habilitation teams and DSP staff
- Bilingual candidates encouraged to apply
