Description of the Role:

As a Physical Therapist on our Home Health Team at Interim HealthCare you will have the opportunity to make a significant impact on the lives of our patients. Whether you choose to work full time or part-time, you will be responsible for providing high-quality care and personalized treatment plans to our patients in the comfort of their own homes. This role is currently for Per-Diem with potential to transition to part-time or full-time.

Responsibilities:

• Conduct comprehensive evaluations of patients' physical conditions and develop individualized treatment plans.
• Implement treatment plans and monitor patients' progress closely, making necessary adjustments as needed.
• Provide and instruct on therapeutic exercises, therapy techniques, and other interventions to improve patients' mobility, strength, and overall function.
• Educate patients and their families on the importance of compliance with treatment plans and home exercise programs.
• Maintain accurate and up-to-date documentation of patients' progress, treatment plans, and any changes in condition.

Requirements:

• Valid Physical Therapy License in the state of Pennsylvania.
• Excellent clinical skills and knowledge of evidence-based practices in physical therapy.
• Strong communication and interpersonal skills to effectively collaborate with patients, their families, and other healthcare professionals.
• Ability to work independently and prioritize tasks in a fast-paced environment.
• Experience in home health or a similar setting is preferred.
• Familiar with Oasis Documentation requirements
