Licensed Practical Nurse

- Graduate of an approved school of nursing; registered nurse or LPN with a current license to practice in the Commonwealth of Pennsylvania.
- Experience with or knowledge of acceptable assessment instruments used in evaluation of human growth and development.
- Experience working with individuals with intellectual and physical disabilities preferred.
- Valid PA driver’s license.
- Provide a supportive, caring, safe environment by attending to individual needs, respecting differences, and supporting growth and independence.
- Training and supervision of medical plan of care.
- Involvement in referral process (intake, assessment, placement, transfer) and supervision of referrals as requested.
- Preparation of required or requested reports.
- Attend meetings as required and act as a member of the Interdisciplinary Team.
- Evaluation and monitoring of medical needs of program participants to ensure that appropriate health services are provided in collaboration with program staff.
- Coordination of scheduling and attendance at individuals’ medical appointments as needed.
- Complete assessments of program participants and analyze information to develop
- Administer medications and treatments as needed to program participants; assist
- Assess program participants upon admission or re-admission to develop interim care
- Assess the program participant’s condition routinely and as needed based on health
- Assist with maintaining program participant clinical files; keep charts updated;
- Assist with HRST rating as requested and complete nurse reviews in a timely
- Review controlled substances count sheets; inventory and review medication
- Communicate status of program participants according to established procedures;
- Provide on-the-job training, evaluate performance, and provide direction.
- Regular attendance at work.
- Other duties as assigned.
