We are looking for an experienced and motivated Nurse Manager to supervise a team of Medical Assistants in delivering care management services to include Transitional Care Management (TCM), Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and other preventative care programs. This nurse will be responsible for managing a panel of patients, overseeing patient care workflows, and driving performance through key clinical metrics. The ideal candidate is someone who thrives in a fast-paced, tech-enabled healthcare setting and is passionate about patient outcomes and team development. Responsibilities include:
Patient Care Management
- Manage a personal caseload of patients by providing regular touchpoints and care management services to patients with complex needs or transitions of care.
- Conduct clinical assessments, develop care plans, help patients navigate the healthcare system, and coordinate patient access to necessary services and resources.
- Manage medical care and treatment by coordinating high-quality health care and support, implementing appropriate interventions, and coordinating follow-up care.
- Assess patients' physical, psychological, and social needs, monitor patient progress, and provide education and support to patients and their families.
- Execute care management services for patients identified at the highest risk for complications, readmissions, adverse events or exacerbation, with the goal of decreasing unnecessary emergency department visits/hospitalizations.
- Oversee Transitional Care Management (TCM) for all patients discharged from in patient facility ensuring interactive contact, Primary Care Provider visit, DME and Home Health Services, specialist appointments, and community resources/social services are provided as indicated.
- Oversee other programs including value-based clinical initiatives, chronic care management, ACO programs, and other CMS care management services.
Team Leadership
- Supervise a team of Medical Assistants delivering remote proactive care services.
- Review care plans, documentation, and patient notes for quality and compliance.
- Work collaboratively with members of the care team to provide comprehensive care management services.
- Participating collaboratively in the achievement of quality measures; health education activities; quality improvement meetings; and successful audits.
- Demonstrate strong leadership, management, and interpersonal skills to accomplish hiring, training, performance management, frequent team check-ins, provision of clinical guidance, and skill development.
- Triage and respond to clinical escalations from MAs or monitoring alerts.
- Communicate with providers and patients to resolve clinical concerns promptly.
Program Oversight
- Ensure adherence to clinical protocols, documentation standards, and CMS guidelines.
- Collaborate with others to streamline workflows and improve efficiency.
- Lead clinical quality assurance and documentation audits.
- Analyze data to identify and implement population health needs for various patient groups; guide coaching, identify training needs, and support strategic planning; and work cross-functionally with leadership to present findings and design program improvements.
- Track and report on program key performance indicators (KPI) such as enrollment, retention, engagement, and outcome measures.
Requirements:
- Graduate of an accredited nursing program
- Active RN license in the Commonwealth of Pennsylvania
- Minimum of 2 years clinical experience in an ambulatory care setting
- Preferred: Knowledge of Medicare programs (TCM, CCM, RPM)
- Preferred: Project management skills
- Strong oral and written communication skills required
- CPR certification required
