Position Overview:

Juvenile Forensic Service Coordinators provide specialized service coordination to 25-30 youth (ages 10 – 18) involved with Juvenile Justice System. Service Coordinators complete assessments, individualized service plan and crisis plan, treatment referrals, and connections to community resources to address social determinants of health. Staff closely collaborate with Juvenile Justice System to support clients with meeting the expectations of the court. Successful service coordinators build trusting relationships with youth and families, maintain timely documentation, meet program utilization, coordinate multidisciplinary care, and achieve service plan goals.

Key Responsibilities:

Client Support and Case Management

  • Serve as the Single Point of Accountability for clients and families, acting as the primary contact and advocate for services.

  • Build and maintain enduring relationships with clients and their families through persistent outreach and engagement.

  • Develop comprehensive service plans tailored to individual strengths and needs, regularly updating and presenting progress to the court.

  • Support clients during times of transition, including those preparing for return to community from treatment programs, detention facilities, and hospitalization.

  • Coordinate care across various providers, including behavioral health, juvenile probation, child welfare, drug and alcohol services, schools, and community resources.

  • Maintain monthly face-to-face contact with clients, ensuring services are provided in line with program standards. Meet with clients throughout Allegheny County, including homes, schools, treatment programs, courts, juvenile probation placements, detention centers, county jail, etc.

Collaboration and Advocacy

  • Work closely with families, mental health services, probation officers, educational staff, and community providers to address barriers to service plan goals.

  • Facilitate interdisciplinary team meetings, ensuring follow-through on action steps and ensuring continuity of care.

  • Act as an advocate for clients, helping remove systemic barriers, resolving issues, and fostering full inclusion within their community.

  • Attend school-based meetings and offer educational advocacy as needed, ensuring clients have the support they need to thrive.

Documentation and Compliance

  • Complete assessments, service plans, and case notes within specified timeframes, ensuring all documentation complies with state and agency regulations.

  • Maintain accurate records of all interactions, including family meetings, service linkages, and follow-up activities, in electronic health records.

  • Meet monthly utilization expectations.

  • Comply with HIPAA confidentiality policies and participate in regular audits and program evaluations.

Professional Development

  • Participate in ongoing training and development opportunities, staying updated on best practices and trends in behavioral health, juvenile justice, education, and service coordination.

  • Actively utilize individual and group supervision to triage clients, monitor trends, enhance the program.

Required Skills and Qualifications:

  • Bachelor’s degree in Social Work, Psychology, Education, Criminal Justice, Political Science, or a related field (Master’s degree welcome).

  • Two years of experience in a behavioral health, juvenile justice, or human service setting.

  • Strong organizational skills with the ability to manage multiple priorities and meet deadlines.

  • Excellent communication and interpersonal skills, with the ability to work independently and as part of a multidisciplinary team.

  • Demonstrated cultural competence and the ability to work with diverse populations.

Additional Requirements:

  • Valid Pennsylvania driver’s license with access to a reliable vehicle.

  • Current auto insurance that meets agency requirements.

  • Act 33, Act 34, FBI Clearances, Institutional Clearances.

  • Ability to complete required onboarding training within 30 days of hire.

Work Environment and Benefits:

  • Full-time position (40 hours per week), with occasional evening or weekend hours to meet client needs.

  • Hybrid work model: Field-based position with the option to complete administrative tasks remotely after consistently meeting program standards.
