Behavioral Health State Hospital Liaison
Essential Duties and Responsibilities
- Participates/attends monthly treatment team meetings/commitment hearings for individuals on a mental health civil commitment who are Allegheny County residents being served at the Torrance state hospital.
- Facilitates Community Support Planning (CSP) meetings as the Office of Behavioral Health (OBH) representative for discharges from the state hospital, as well as participate in any needed planning meetings for individuals at both the Torrance civil and forensics units on an ongoing basis.
- Participates in quarterly meetings with Torrance and the contributing counties regarding census data and system updates.
- Coordinate with Allegheny County service providers to assist in the submission of any needed housing and/or service referrals for individuals.
- Maintains and oversees the census data for Torrance civil.
- Completes a monthly report for individuals at Torrance civil and actively works with the treatment team to facilitate discharge planning as soon as possible for each individual at civil.
- Participate in the ongoing the Torrance Sexual Responsibility and Treatment Program ( SRTP) meetings for any Allegheny County cases, as well as any needed ongoing court hearings/reviews for those individuals.
- Assist as needed with the identification of resource options and supports to assist individuals being released by the SRTP.
- Travel required to the Torrance State Hospital on a routine basis.
- Serves as the OBH representative for the monitoring of the Dual Diagnosis Treatment Team (DDTT) program, acts as a liaison with Mental Health (MH) and Intellectual Disability (ID) systems.
- Back up facilitator for the OBH Disposition Coordinator by covering meetings as needed.
- Assists the Disposition Coordinator with organizing/processing the Dispo notes into the system.
- Assists the unit with identified MH housing coordination as needed in relation to assigned projects and tasks as they come up.
- Assists the unit with maintaining and reviewing updated and current wait lists in the MH housing database.
- Assist the unit in housing monitoring duties as needed.
- Additional assignments as they come up may be assigned.

- Considerable knowledge of overall operational policies and established procedures in the community, institutional or facility-based programs.
- Ability to establish and maintain effective working relationships with patient, clients, other members of the treatment team and public and private agencies.
- Ability to communicate effectively, both orally and in writing.
- Ability to maintain necessary records and prepare reports.
- Ability to maintain diplomacy and tact to problem solve in a constructive manner.
- Ability to work independently with minimal direction.
- Excellent organizational skills.

Education/Experience Requirements Master’s degree in human services or related field, plus at least 1 year related experience. - OR- Bachelor’s degree in human services or related field, plus at least 3 years related experience.

Any equivalent combination of education and experience that meets the required knowledge, skills and abilities. Certificates, Licenses, Registrations (List Act 33, 34 and/or FBI clearances required, depending on Job Description.)
