Responsibilities:

What You’ll Do:

• Conduct patient assessments and create individualized care plans.
• Deliver top-quality clinical care and monitor progress.
• Collaborate with LPNs, therapists, and home health aides to ensure holistic care.
• Educate patients and families on health conditions and care techniques.
• Provide compassionate, patient-centered nursing in a home health setting.

Qualifications:

What You Bring:

• Active RN license in Pennsylvania.
• Minimum of one year of nursing experience (home health preferred, not required).
• A commitment to quality, compassion, and clinical excellence.
• Strong communication and critical thinking skills.
• Ability to work independently with confidence—and the full support of your team.
