JOB DESCRIPTION - PNR SPEECH THERAPIST (SLP) REPORTS TO: Operations Manager
JOB SUMMARY:
Evaluates, plans, and administers comprehensive speech therapy modalities/regimens as prescribed by a physician to patients with speech, voice, and language, cognitive communicative and oral-pharyngeal swallowing disorders in their place of residence for the purpose of improving and/or restoring speech and/or swallowing functions.
DUTIES AND RESPONSIBILITIES:
Performs accurate and comprehensive initial and ongoing speech, language and swallowing assessments in response to patients’ needs and in accordance with physician orders.
Initiates a written plan of care, re-evaluates and updates the plan as necessary.
Provides skilled physical therapy modalities within the therapy scope of practice.
Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures.
Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, to assure patient care needs are met.
Addresses and supports patient/family cultural practices, as long as such practices do not harm others or interfere with the planned course medical therapy.
Promotes and provides patient/family/caregiver education using various verbal and written communication techniques that take into account the patient’s/family’s cultural, ethnic, and/or personal needs or preferences.
Supervises Speech Therapy Assistants in compliance with applicable laws, regulations and standards.
Plans patient discharge appropriately; provides information about community resources to address patient ongoing needs.
Knowledgeable of federal, state regulations and Agency’s policies and procedures regarding patient care. 215 Solutions for Care, Inc.(c) 2020
Communicates with all members of the health care team, including physicians appropriately and in a timely manner.
Notifies the physician, the Clinical Manager and other health care team members of any changes in the patient’s condition and the need to modify the plan of care.
Maintains currency of knowledge and skills.
Participates in the Agency’s Quality Assessment Performance Improvement program.
Attends Agency meetings and in-service programs as required.
Obtains appropriate number of continuing education credits to maintain re-licensure status.
Maintains confidentiality of patient and Agency information at all times.
QUALIFICATIONS/PROFESSIONAL REQUIREMENTS:
Master’s Level Graduate in the area of Speech Therapy or Speech Language Pathology from an accredited university of college.
Certificate of Clinical Competence in Speech-Language Pathology awarded by the American speech Language and Hearing Association.
For Medicare Certified Agencies:
Meets the education and experience requirements for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech-Language-Hearing Association; or
Meets the education requirements for certification and is in the process of accumulating the supervised experience required for certification.
Current valid state Speech Therapy license
Minimum of two (2) years’ experience as a Speech Therapist
Current CPR card
Current health certificate/physical examination and TB testing results
Valid state driver’s license and reliable automobile
Proof of current automobile insurance
Able to communicate effectively in English, both verbally and in writing. 216 Solutions for Care, Inc.(c) 2020

PHYSICAL DEMANDS:
While performing the functions of this job the employee is frequently required to stand, walk and sit, and use hands, use fingers, handle or feel.
The employee must be able to lift and/or move up to 50 pounds.
