About the Role:
The Certified Registered Nurse Practitioner (CRNP) plays a critical role in delivering high-quality, patient-centered healthcare AT White Horse Village. This position involves conducting comprehensive patient assessments, diagnosing acute and chronic conditions, and developing individualized treatment plans to promote optimal health outcomes. The CRNP collaborates closely with physicians, specialists, and other healthcare professionals to ensure coordinated and effective care delivery. Additionally, the role requires educating patients and their families on health management, preventive care, and treatment options to empower informed decision-making. Ultimately, the CRNP contributes to improving community health by providing accessible, evidence-based medical services and advocating for patient well-being.
Minimum Qualifications:
- Current and unrestricted Registered Nurse (RN) license in Pennsylvania.
- Master’s or Doctoral degree in Nursing from an accredited program.
- Certification as a Nurse Practitioner (NP) from a recognized national certifying body.
- Clinical experience in a relevant healthcare setting, preferably with geriatric and direct patient care responsibilities.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
Preferred Qualifications:
- Experience working in primary care, family practice, or specialty clinics.
- Additional certifications relevant to specialty areas such as geriatrics or acute care.
- Familiarity with electronic health record (EHR) systems.
- Strong background in patient education and community health initiatives.
- Demonstrated leadership or mentorship experience within clinical teams.
Responsibilities:
- Perform thorough physical examinations and obtain detailed patient histories to assess health status.
- Diagnose and manage acute and chronic illnesses, prescribing medications and therapies as appropriate within scope of practice.
- Order, interpret, and follow up on diagnostic tests such as lab work and imaging studies to inform clinical decisions.
- Develop and implement individualized care plans, including health promotion and disease prevention strategies.
- Collaborate with interdisciplinary healthcare teams to coordinate patient care and ensure continuity.
- Provide patient education on treatment plans, medication adherence, lifestyle modifications, and preventive health measures.
- Maintain accurate and timely documentation of patient encounters, treatment plans, and clinical outcomes.
- Participate in quality improvement initiatives and stay current with advances in medical knowledge and best practices.

The CRNP utilizes advanced clinical assessment and diagnostic skills daily to evaluate patient conditions accurately and develop effective treatment plans. Strong communication skills are essential for educating patients and collaborating with multidisciplinary teams to ensure comprehensive care.
