Community Care Worker
The C.H.A.P.E.L. (Community Health Assistance, Programming & Engagement Leadership) program, an outreach program of Bible Center Church and a Second Chance, Inc. is seeking a dedicated and talented Community Care Worker to increase community awareness of mental health signs and symptoms through education, outreach, and non‑threatening access to services. This role plays a vital part in strengthening community wellness by building trusting relationships, connecting individuals to resources, and promoting mental health literacy in a culturally responsive way.
Job Title: Community Care Worker (CCW)
Reports To: Director of The CHAPEL Program
Position Type: Part‑Time, Non‑Exempt Hours: 20 hours per week
Compensation: $22.50 per hour

Key Responsibilities
- Schedule and conduct regular appointments with program participants
- Build supportive, empathetic relationships with individuals and families
- Provide information, referrals, and access to appropriate community and mental‑health resources
- Participate in required training and ongoing professional development
- Maintain confidentiality and adhere to all program, organizational, and legal standards
- Submit documentation, reports, and case notes accurately and on time
- Support community outreach events, awareness campaigns, and educational activities
- Maintain a valid driver’s license, reliable transportation, insurance, and an in‑service mobile phone
- Obtain and maintain all background clearances required by the Commonwealth of Pennsylvania and Child Protective Services Law

- Willingness and ability to work with clients from diverse cultural, socioeconomic, and lived‑experience backgrounds
- Strong communication and interpersonal skills
- Computer Software usage—Microsoft Word, Email, Zoom,
- Ability to maintain professionalism, boundaries, and confidentiality
- Reliable, organized, and able to manage time effectively
- Comfortable working in community settings and engaging with the public
- Commitment to learning about mental health, trauma‑informed care, and community resources
- Ability to work independently and as part of a collaborative team
