Were supporting a respected organization in the Lehigh Valley/Bethlehem region that is adding a Home Health & Hospice Patient Navigator to strengthen their community presence and ensure patients and families know exactly where to turn for compassionate care.
This role is perfect for someone who genuinely enjoys connecting with people, building trust across referral networks, and representing a mission-driven organization with professionalism and heart.
What This Role Really Looks Like
You'll be the face and voice of the agency out in the community, inside hospitals, visiting physician offices, supporting discharge planners, and helping educate families about the care they can receive at home. Every day is different, but the purpose is consistent: making sure patients get the support they deserve.
You will:

Build relationships with physicians, hospitals, social workers, discharge planners, senior care partners, and community organizations.

Share information about home health and hospice services in a clear, compassionate way.

Make regular visits to referral sourcesanswering questions, delivering materials, and reinforcing trust.

Attend community events, health fairs, and networking functions to increase visibility.

Track referral activity, leads, and market trends in the CRM.

Work closely with clinical leadership to ensure messaging aligns with the care delivered.

Provide ongoing insights to leadership about opportunities, gaps, and referral-source needs.

Assist with brochures, outreach materials, and education campaigns as needed.
What You Bring
- Bachelors degree in Marketing, Business, Healthcare, or a related field (preferred).

3+ years of healthcare sales or marketing experience (required).

Direct experience in home health or hospice (required).
- Strong understanding of home health/hospice care, referral workflows, and compliance expectations.

Excellent communication and relationship-building skills.
- Ability to work independently, manage priorities, and thrive in a fast-paced community setting.
- Comfort engaging with a diverse group of patients, families, and healthcare partners.
Compensation & Schedule

$60,000 $80,000 annual salary , depending on experience
Industry-leading commission-based referral structure on top of base salary
