Job Summary:
The Home Health RN Flex provides skilled and compassionate nursing care to patients in their homes in compliance with physician orders, Medicare Conditions of Participation, Pennsylvania licensure laws, and agency policies. This role primarily supports patient admissions (Start of Care and Resumption of Care) and revisits, ensuring safe, high-quality care. The RN Flex collaborates closely with the interdisciplinary team to coordinate and deliver care that aligns with the agency’s mission of providing exceptional healthcare in the home and our vision to be the regional home health and hospice agency of choice.
Primary Responsibilities:
Clinical Care & Nursing Process:
- Conduct thorough, accurate, and timely patient assessments.
- Collaborate with physicians to plan and implement individualized care plans.
- Provide nursing interventions, preventative and therapeutic procedures, and evaluate outcomes.
- Facilitate patient and family education to promote continuity of care and positive outcomes.
- Document complete, accurate, and timely clinical notes in compliance with agency, state, and federal requirements.
- Meet productivity expectations as determined by patient acuity and Clinical Manager.
Education & Mentorship:
- Act as a mentor to staff by participating in orientation, field training, and in-services.
- Teach patients, caregivers, and families regarding disease processes, medications, nutrition, and self-care.
- Supervise Home Health Aides every 14 days per Medicare regulations.
- Promote ongoing professional development of staff to enhance patient outcomes.
Leadership & Collaboration:
- Demonstrate behavior consistent with the agency’s mission, vision, and values.
- Exhibit creativity, critical thinking, and leadership in case conferences and patient care planning.
- Function independently, managing time and assignments effectively.
- Coordinate with the interdisciplinary team to ensure comprehensive care delivery.
Communication:
- Introduce self and clarify role with patients, families, caregivers, and referral sources.
- Maintain confidentiality in patient matters.
- Communicate effectively with all team members, providing input in conferences, meetings, and reports.
- Offer feedback, suggestions, and information constructively and professionally.
Resource Utilization:
- Coordinate travel efficiently and cost-effectively, understanding Home Health reimbursement practices.
- Manage supplies, equipment, and records appropriately.
- Participate in evening/weekend on-call rotation as required.
- Maintain compliance with Medicare Conditions of Participation, Pennsylvania licensure laws, and agency policies.
Customer Service Excellence:
- Demonstrate the highest level of customer awareness and responsiveness.
- Treat all patients, caregivers, and staff with dignity and respect.
- Provide care in a friendly, empathetic, and professional manner.
- Maintain accountability for patient satisfaction throughout episodes of care.
Performance Improvement:
- Participate in performance improvement initiatives to enhance patient outcomes.
- Assist with clinical record reviews, committees, and projects.
- Contribute to continuous monitoring of quality, compliance, and safety.
Secondary Responsibilities:
- Provide nursing visits to the VNA Hospice program as needed.
- Attend staff, departmental, and interdisciplinary meetings.
- Participate in committees and projects as assigned.
- Perform other duties as requested.
Physical Requirements and Working Conditions:
- Strong verbal and written communication skills.
- Visual and hearing ability sufficient to comprehend communication.
- Manual dexterity for documentation and telephone use.
- Ability to work in varied home environments and conditions.
- Valid PA driver’s license, reliable transportation, and automobile insurance.
- Able to sit, stand, walk, bend, lift, and carry up to 25 lbs. See attached Physical Requirements form.
Qualifications:
- Graduate of an accredited nursing program; BSN preferred.
- Current Registered Nurse (RN) license in Pennsylvania.
- Minimum one year of nursing experience; home health, medical/surgical, or critical care experience preferred.
- CPR certification.
- Satisfactory criminal background check.
