Youth Support Partner
The High-Fidelity Wraparound process; known in Delaware County as High Fidelity Family Teams, is a needs based planning process to improve the lives of children with complex mental health needs and their families by empowering the families and youth to have their voices heard through an individualized planning process.
The individualized plan is child-centered and family-focused with the process focusing on strengthening the natural family, extended family and social supports for the child and family by developing a team to support the family throughout the process and in learning the skills of the model
Please note, that in order to be a Youth Support Partner, it requires you to have had personal involvement with a child-serving system. WORK-RELATED EXPERIENCE DOES NOT COUNT.Job Overview:
The Youth Support Partner is a full-time position that works with the youth (and families during meetings) throughout Delaware County area who have agreed to participate in the High Fidelity Family Teams Process.
The Youth Support Partner will work as part of the High Fidelity Team providing support and guidance to the youth throughout the process, during meetings, and in completion of the individualized plans developed by the youth and their family. The Youth Support Partner will work with the identified youth to develop their own team of supports (natural, community, and systems).
The Youth Support Partner will use their own life experiences to model for the youth how to navigate systems using their voice to meet their needs while developing natural supports within their own community to reach their family vision.
Required Qualifications:
- Have prior experience handling complex emotional and behavioral needs.
- Have life experience which allows you to possess a diverse understanding of the struggles youth with complex emotional needs endure.
- Have successfully navigated child serving systems (mental health, child welfare, probation, education)
Compensation and Benefits:
- Hourly rate of $17.00
- Annual Salary Increase and Bonus
