SUMMARY OF POSITION:

Responsible to provide clinical management for Endocrinology and Nephrology high/at risk patients. They would work with the Endocrinology and Nephrology Attendings and APPs in clinic. The Clinical Case Manager will focus on working with advanced CKD patients that are not on dialysis to manage and educate high/at risk patients including their medications and blood sugars. Works with the medical directors as point person, with urgent consult requests, and timely hospital follow ups in clinic. Works with social worker to set up home health care. Assists with prior authorization for DME equipment and form completion.

MINIMUM QUALIFICATION(S):

• Two (2) years nursing experience

• Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.

• AHA BLS prior to end of orientation period
